Fill out your new patient and medical history forms (print or digital) before your visit.

Bring your BC Services Card and a piece of photo ID if available.

Bring your insurance provider name and policy/member information (or your benefits card).

Include prescriptions, over-the-counter medications, and any allergies.

Ask us for the forms by email or pick them up at the clinic.

Take your time and provide the most accurate medical and dental history you can.

Email your completed forms to info@pearlcaredental.ca or bring them to your appointment.

We use your information to provide safe care and coordinate benefits when applicable.
